What is the Line Note on the Add/Edit Session Form?

A Line Note is required when a CPT code is considered non-specific or does not definitively describe the service/supply.

According to the ASCX12-5010 implementation guide for EDI claims, a non-specific code may include in their descriptors terms such as:

• Not otherwise classified (NOC)
• Unlisted
• Unspecified
• Unclassified
• Other
• Miscellaneous
• Prescription Drug
• Generic
• Prescription Drug, Brand Name

When entered onto a session, it will be placed in Loop 2400, Segment SV101-7 of the EDI file. On a CMS-1500, the Line Note will populate the shaded area of Box 24. Entering a Line Note when not required can result in a rejection from the payer.
