Sub-element CLM11-04 is missing. It is required when CLM11-1,-2, or -3 = 'AA'. Segment CLM is defined in the guideline at position 1300.

This error is referring to the auto accident indicator field.  If you choose Yes to indicate that this claim is being filed for a patient whose injury is due to an auto accident, then you must supply the state that the accident occurred in.

To update this information in Therabill, follow these instructions.

1. Open up the edit client form (if you are on the claim error page in Therabill, then you can do this by clicking the edit icon next to the client's name on the right of the page).
2. At the top right of the edit client form, click the Advanced tab.
3. At the bottom left of the advanced page, next to where you indicate Yes for Auto Accident you will see a text box labeled State.  Enter the two letter abbreviation for the state where the accident occurred in to that text box.
4. IMPORTANT:  Save the client.
