500 - Entity's Postal/Zip Code. Usage: This code requires use of an Entity Code.

NOTE: If you receive this error, you may also want to check the Zip code that you have on file for the patient.

This occurs when either the Zip code that you have entered is invalid or does not match the state that you provided.  You can edit the Zip code for the patient from the edit client form in Therabill.

IMPORTANT: If the patient is also the insured, you will also want to make the change on the insurance card.  You can edit the insurance card by clicking the edit icon next to the insurance card on the bottom right of the edit client form.

If you are on the on the claim error page in  Therabill, you can quickly edit the client information by clicking the edit icon next to the client name on the right of the page.  After you save your changes, you can then click the Resubmit Electronically button at the top of the error page. 

Error Variations

• Value of element N403 is incorrect. Expected value for ZIP Code is 9 digits. Segment N4 is defined in the guideline at position 2700.
• This Zip Code is not valid for this State
