Payer Specific Rejection: CPT/HCPCS code used requires Referring physician (HCFA Box 17, UB Box 78)

The insurance company that you are submitting to appears to require a referring physician on the claim when the service code (CPT code) that you are submitting is used.

How to Fix

You will want to enter the referring physician on the client record in Therabill.

• Click Clients in the top navigation bar of Therabill and then find the client in your client list.
• Click the edit icon associated with the client to open up the client record.
• On the demographics page, under Referring Physician, enter the referring physicians NPI, first name and last name.  IMPORTANT:  NPI is required for insurance claims.
• Save the client.
