The payer is rejecting stating that either the full name, the identifier (most likely the primary ID you have entered on the insurance card in Therabill), the patient gender, and/or the date of birth do not match a subscriber/dependent that they have in their system.
Make sure all of the information you have entered for the client in Therabill matches the information on the insurance card. Make sure the date of birth and gender you have entered for the patient are correct and match what the payer has on file for the patient.
