The payer is rejecting stating that the last name for the subscriber is missing (or invalid) on the claim. Make sure you have the last name entered on the insurance card in Therabill. Make sure the last name that you have entered matches what is on the insurance card you received from your patient. Make sure you have only entered alphabetic characters (some payers will reject if you have non-alphabetic characters for the name).
