The payer is rejecting based on some characters that you entered in the referring provider name (this rejection message specifically mentions the last name).
Go to your Referring Provider list (put your mouse over Providers in the top navigation bar and then click on Referring Providers in the drop down menu). Find the referring provider on this claim and click the edit icon associated with the referring provider. In the edit referring provider form, make sure you do not have any special characters (while they specifically say numeric, they are more than likley referring to non-alphabetic characters).
The last name should only contain the referring providers actual last name.
- Do not put the physicians credentials as part of the last name (put them in the credentials box).
- Do not put any codes (that you might use internally) as part of the name (as it will go out on your claims).
