Unfortunately, the payer is just saying that they are returning/rejecting the claim but they are not giving any specific details as to why. You will need to call the payer to find out why they are rejecting your claims.
IMPORTANT: When calling the payer, if they say that they do not have any record of receiving your claim, let them know that you submitted it electronically and it was rejected by their Electronic Data Interchange (EDI) department.
