The payer is rejecting because you do not have the insured's name (first name, last name or both) on the claim.
You will want to edit the insurance card in Therabill to make sure you have the first and last name of the insured entered on the insurance card. Once you have verified this, you can resubmit this claim from the EDI rejection page in Therabill by clicking the Resubmit Electronically button at the top of the page.
