This rejection error:

Indicates an Invalid Remark Reason code was applied to the adjustment for this insurance payer. You will need to delete the adjustment and post a new adjustment with a valid remark code.
To Resolve
Follow the steps below to delete the adjustment then post a new adjustment.
Delete the adjustment
You can use the Single Session or Search Payment method to delete the adjustment.
Delete Adjustment - Single Session Method
Use this method, if you know the Date of Service for the adjustment you need to delete.
- From the Therabill Dashboard go to Payments, Enter Payments then click Single Session.
- You can find the adjustment session using the filter when you have the client’s name or Date of Service.
- Once you find the session, click the (dollar sign icon) to open the Single Session Payment Form.
- All payments and adjustments applied to this session are listed in the Payment History section. Find the adjustment and click the (delete) button.

Delete Adjustment - Search Payments Method
Use this method, if you Do Not know the Date of Service for the adjustment you need to delete.
- From the Therabill Dashboard go to Payments then Search Payments.
- Use the available Payment Search filters to find the adjustment.
- Once you find the adjustment session, click the (delete) button.

- Click Delete, to confirm you want to delete the adjustment and remove it from the client record.

Post New Adjustment
Now you will need to post a new adjustment ensuring you have entered a valid therapy remark code.
- Go to Payments, Enter Payments, then click Single Session.
- From here, enter the client name into the Client/Patient field and select the name from the drop-down menu. Also, you may need to extend the beginning date on the Service Date Range to include the needed Date of Service.
- Click the icon that corresponds with the Date of Service to open the Payment window.
- From the New Payment/Adjustment area, use the Type drop-down menu and select Insurance w/ Adjudication Info.
- If necessary update the Received from field to be the Primary Insurance payer. Tip: Once you begin typing the Payer List will pop up, then you can click on the correct payer.
- From the Payment & Adjustment Entry section, find the Service Line.

- Complete the New Adj dollar amount.
- Fill in the correct Remark code and ensure you click on the correct code description from the pop-up list to add the code to the box.
- Check the Post Adj box, to allow the adjustment to be posted to the session and appear on the claim transmission. ds"> in this article to remap the insurance.

- Use the Action drop-down and select Keep Current Session Status.
- Once all CPT codes have been updated, click Submit.
