Box 11a - Insured's Date of Birth, Sex

What is it?

Box 11a displays the insured's 8-digit birthdate (MMDDYYYY) and sex.

In Application:

Note: To make this change permanent, you must update this information directly in WebPT. Otherwise, the next time you save in WebPT, the information entered there will flow over and overwrite any changes you've made.

To manually change this information:

1. Navigate to Clients > Client List.
2. Edit the desired client using the icon.
3. Edit the corresponding insurance card using the icon.
4. In the Demographics section, enter the information into the Birthdate and Gender fields.
5. Select Apply Edits.

In WebPT:

To manually change this information:

1. Navigate to the desired patient's chart.
2. Select Patient Info.
3. Click the  icon for the corresponding insurance entry.
4. Use the Next button until you get to the Policy Information screen.
5. Use the Insured Party drop-down to make a selection.
   
   1. If an option other than Self was chosen, select the Next button and enter the information for the Insured Party.
6. Use the Next button until you reach the final screen and select OK.
7. Click Save Patient.

Note: If the Insured Party is Self, the patient's information will be used.
