What is it?
Box 26 is used to indicate the identifier/medical record number assigned by the provider. The Medical Record Number is a unique identifier assigned to each client within the system.
Note: This is for Print & Mail claims only. Electronic claims will always use a unique Claim Identifier in this field.
Where can I find a patient's medical record number?
The medical record number can be found under Clients > Client List.
