This article applies to Therabill's custom documentation system and not to Simple Notes. For information on Simple Notes, please see: Simple Notes in Therabill.
You may have note templates where it is necessary to get the referring provider / physician (or PCP) signature on a specific note template. For example, you may want a signature field for the referring physician on the treatment plan or plan of care, but not on your daily notes. Therabill allows you to decide which note templates will be printed out with a signature field for the referring physician.
How do I get a Referring Physician signature box on my printed note.
You can tell Therabill to put a referring provider signature box on specific note templates. This can be done when you are creating a new note template or you can edit a note template you have already created to include a referring provider note signature box when printed.
Adding signature box when creating a new note template
If you are unsure how to create a new note template from the note template manager, we recommend that you watch one of our recorded webinars on our documentation system. The recorded webinars can be found by logging in to Therabill and clicking the Recorded Webinars link at the top left of your dashboard. This article will assume you know how to copy one note template to create a new note template.
On the copy note template form (see image to the right), where you create a new note template, you will see some signature options (under the section Signatures). One of those options is to Include referring physician signature box on printed note. Put a check in that checkbox before you click the Save button.
Editing a note template to include a referring provider note signature box.
If you have already created a note template and you would like to add a referring physician signature box to the printed note template, then you will need to edit the settings for the note template. To edit the settings for a note template, open up the Manage Note Templates form (this can only be done through an administrative account, that is, the account owner or a user account. This cannot be done through a provider portal). You can get to the Manage Note Templates form by clicking the Manage Note Templates link at the top right of your month view calendar (under Schedule in the top green navigation bar of Therabill). Once you have the edit note templates form (note template manager) open, at the top left, under the drop down menu where you select the note template, you will see a link labeled Edit Names or Remove Templates. Click that link. You will now see a list of all of your note templates. Each note template will have an edit icon associated with it. Click the edit icon. You will now see a form similar to the form shown on the left of this page. In the form, there is a section labeled Signatures. One of the signature options is Include referring physician signature box on printed note (see image to the right, red arrow). Put a check next to that option and then click the Save button at the bottom of the form.
What does the referring provider signature box do?
Whenever you print out the note (in PDF format), Therabill will put a signature line where the referring physician can sign and date the note. Underneath the signature line, a caption will read I certify the need for these services furnished under this plan of treatment while under my care.
I have chosen this option for my note template, but the signature box is not showing up.
Check to make sure that you have entered a referring physician for the client. The referring physician is entered on the add / edit client form at the top right of the demographics page.
